The Table of Contents is not a form. You do not have to have your web published in order for it to work, you can FTP it. However, you will NOT be able to see the pages in the design view nor in the "Preview" mode. In order to see the Table of Contents, you must go to: File > Preview in Browser > Then select your browser.
